It is critical to our success that we work together with other organizations to build healthy communities. However, it can be challenging when we each have different expertise, investment levels, and data sources. How can we unite diverse entities to work toward shared goals for better community health? Maggie Graul, MPH, CPH, will showcase the "High Impact Towns" model from the Monadnock Region, which helps workgroups make data-driven decisions for more equitable health outcomes. They will also share a method for identifying priority areas in your service region with the greatest potential for impact, featuring recent work of Cheshire Medical Center and the Healthy Monadnock Alliance.
